2016-17 School Year Information

Dear Parents and Students,
 
Tam High will not be mailing packets home this year.  You can access all forms here on the website or pick them up in the main office.  Please read through all of the documents below, and print the ones that apply to your student.  Students can return the forms on schedule pickup days, or drop them off in the main office, Wood Hall.  Thank you!  We look forward to a great year.
 
REQUIRED for 10th, 11th, and 12th grade students:  Instructions to update your emergency information online are linked HERE.  This must be completed in order for students to receive their schedule of classes.
 
Principal's Letter: please read this important information about the schedule pick-up process for new and returning students.
 
PAN (Parental Annual Notice):  REQUIRED form for ALL students.  Please print it out, sign it, and return it with each Tam student on schedule pickup day.  If you want to read the full text of the PAN, 49 pages long, please click here.  You do NOT need to print this full text version.
 
Spirit Pack: Items available for sale include Yearbook, school planner, Tam spirit clothing, and more.  You can make purchases on the www.tamhigh.org webstore, or fill out the form (available in August) and bring your check to the schedule pickup days in August.
 
Athletic Sports:  If you plan to participate in sports, read the Athletic Director's letter and check the Athletics tab for important requirements. You must have a doctor's signature and return your clearance form before practicing or playing any sport.
 
Athletic Boosters:  Please visit www.tamighboosters.com for more information. Tam appreciates your support of Booster fundraising efforts.  
 
Photo Order Forms:  If you would like to order school pictures from California Classics, please go to their website at https://caclassics.inter-state.com/Shop and enter the Tam school code 08142SA.  Photo pick up day information is also on this flyer.  Student IDs are free and are given out on schedule pick-up day -- purchasing pictures is optional. Medication forms are required to allow school staff to administer acetaminophen or other medications to students during school hours.  You must have a form on file each year.  A physician's signature on the attached form is required.
 
Tam High Foundation (THF) fliers will be mailed home to parents.  Please review as needed.  Tam appreciates your support of THF fundraising efforts.

Insurance:  Do you want to purchase student accident and sickness coverage through Myers/Stevens?   Please read the attached letter or come to the athletic/main office for a copy of their brochure.  Phone # is 800-827-4695.
 
Mill Valley Police Department Bicycle Registration: Please join us on August 20th and 21st from 10am-2pm at the Mill Valley Depot Plaza for a bicycle registration event. Register your bike with the Mill Valley Police Department! This action may assist law enforcement if your bicycle is ever stolen or lost. Having your bicycle information on file will help us match recovered bikes to their rightful owners.
