NEW! ON-LINE ATHLETIC PARTICIPATION FORM: We encourage all families to use this convenient way to submit your annual Athletic Participation Form (APF) for the 2013-14 school year. Save time and paper! Complete most of it on-line for your specific TUHSD school by clicking the appropriate link below. Please turn in the following to your athletic director's office before the student's first season of sport for the year: a) APF signature page, b) physician's signature page and c) final 8th grade report card (for incoming freshmen playing a Fall sport).
The full text APF is available below. Paper copies may be obtained from the Athletic Director's office at each school (or from the Principal's Administrative Assistant during the summer).
Please be sure to notify the Athletic Director if any information in the APF changes during the school year.
